Purpose of This Template
This Occupational Therapy Initial Assessment Template is designed to support Occupational Therapists in completing a structured, comprehensive assessment in a community aged care settings. It guides the collection and documentation of key clinical information required to understand a person’s functional abilities, environmental context, and support needs.
