1. AT-HM Needs and Evidence Summary Template
The document for when you can't prescribe yet. No approval in place, tier too low, no funding for a trial, no quote available. You know a need exists and roughly what it will take to address it, but you can't name a product. This one-page module drops into your initial assessment report or a clinical note, and the provider extracts it as it stands. Cover page explains when to use it, when not to, and what every field is for.
 
2. AT/HM Prescription Template
Built on the six elements the Department requires in a prescription, structured so a provider can find the decision on page one. One table handles multiple items across both AT and home mods, so a bathroom job with four items is still one document. Includes risk and mitigation with ownership, a wraparound services section with hours, and the technical split between what you prescribe and what the builder is responsible for.
 
3. Quick Reference Guide
The scheme, condensed. Tier structure and what sits in each. How co-contributions work and who pays what. Billing for assessment, trials, technical input and follow-up. Which document to use when, and what stays in your clinical record rather than going to the provider. Written to answer the questions that come up mid-visit, not to be read cover to cover.
 
Both templates are editable Word documents with dropdown fields, worked examples you delete before use, and a cover page explaining the purpose of every section. Branding neutral, so add your own letterhead.
