Purpose of this document
This prescription sets out the assistive technology and/or home modifications recommended for a participant, the clinical reasoning supporting them, and what is required to implement them safely. It provides a Registered Provider with the information needed to proceed with purchase or works, and where additional funding is required, the evidence an aged care assessor needs to consider a support plan review.
 
Use it when
The assessment needed to recommend a defined item or modification is complete.
Funding is already available or approved and the provider requires a prescription.
The current approval or tier is insufficient for the selected solution and a quote is available.
A provider requests a prescription for an item at their discretion, including items classified as lower risk or under advice.
 
Why use this template
The Department identifies six elements for a prescription: participant information, prescriber information, a description of the specific AT or HM recommended, justification for the recommendation, wraparound services required, and signatures and dates. This template follows those headings and adds targeted prompts for clinical reasoning, options considered, risk management and implementation responsibilities.
 
It is structured so a provider can identify the decision required on the first page, and a non-OT assessor can follow why the recommended solution is required and proportionate. It supports defensible documentation without reproducing the full OT assessment. It is not intended to duplicate information already documented or to replace the clinical record, where raw assessment data, full trial records and broader clinical information may remain.
